Request an appointment for a CDL/DOT physical or School bus Driver Physical. Also schedule this item for a new driver permit physical. If you are scheduling for a physical, please bring any corrective lenses for reading or driving, hearing aids, a list of your medications, a hemoglobin A1C from the last 3 months (if you are diabetic) and a CPAP or BiPAP readout of the last 3 months ( if you have sleep apnea). If you are unsure of what items you need to bring with you, please call the office to confirm with our staff.
